Default Bad Interface Design

The worst interface design I see these days is on DVDs. For example, I rented a DVD that contains several episodes of a TV series. On the menu, it is not clear which episode is selected. One is bordered in an orange/red color, and the other in red. How am I supposed to tell which one is active. So I have to examine all borders and figure out which one is the odd ball. But on other DVDs with only two choices, it is impossible to tell which one is active.

Also, on the DVD I have now, half the the episodes don't appear on the menu. And there is no indication that these other episodes exist. I stumbled on them by accident. And this is not an unusual situation. I see these problems too often.
