Default Where's my custom 404???

Does anyone know if a unix/linux host handles a custom 404 (404.html) differently than a windows-based webhost?

My custom 404 doesn't work--basically, I enter my domain url with a nonexistent page address "a.html" and I don't get my custom 404 page, I get:

Not Found
The requested URL /a.html was not found on this server.
Apache Server at
[mydomainname] Port 80

My 404.html is at the same file level as my homepage index file which I think is the correct place. If I ref my 404.html directly, the page displays but that sort of defeats the idea...'-}}
