Default PDFs in WinXP Thumbnail View

Sometimes I get no thumbnail for my PDFs when I use WinXP Windows Explorer, "thumbnail" view. I get a generic "A" pdf instead. I use the same process for creating these PDFs:

1. Open blank Word doc

2. Import one jpg image of an online form (4x6 inch)

3. Below it, import one jpg image downloaded from digital camera (4x6 inch)

4. Print page to Acro 6 printer

Sometimes the resulting PDF will have a thumbnail visible in WinEx, sometimes just a "A"dobe icon instead.

Can somebody maybe help me by telling me what determines whether the Windows Explorer can create a thumbnail or just an icon for PDFs?
