Default InDesign colour black printing issue

We recently sent a job to print with a black (C=0, M=0, Y=0, K=100) block covering an image. For some reson the black layer did not cover the picture as it was designed and the picture shone through, making the black darker where the image was.

Is there any explanation for why the image that was on the back layer should shine through?
