Default First line and letter formatting

There is nothing on a test page as yet, but this is another fine example of me complicating matters...

Inside a div class "item" I intend to type several paragraphs, I want to style the first line of the first paragraph differently from the rest of that paragraph and all paragraphs after it. In addition to that I want the first letter of the first paragraph in a different style than the first line!

For example, by declaring:
Code:
p:first-line {font-weight:bold;}
p:first-letter {background-color:red;}
I can get the desired effect on a paragraph, but it's on every paragraph.

I'm thinking that by using a heading styled as the paragraph I can do it by applying the first-line and first-letter styling to that heading, but is there a more elegant solution to this than what I've come up with?
