Default ID3: Double image at the press?

Where I work we have come across a problem 3 times now. On the cover of our job in InDesign 3.0 we place a photo. Everything appears fine, we create a PDF file and send to the printer to print the job. The file on their end is showing a double image, one stacked on top of the other.

Like this when on our end it only shows one Image.

Have any of you ever experienced the same issue and were you able to figure out what happened?

Thanks,
Chris
