There was an earlier thread on this forum about this, which included links to an Adobe "How-to" guide.
From what I can remember, you need to divide each chapter into separate documents (perhaps within a "book"); just have one big text frame with everything in it, including images embedded in the text.
No headers or footers.

That's stretching the boundaries of equivalence, isn't it...?
