CallerID. And if they don't have callerID, GVoice can force them to speak a message saying who they are--and that message is what you hear before the call is connected, or dumped, as you choose.

You can have GVoice simply forward a call, so that you pick it up as soon as you pick up a phone, or you can have the GVoice "receptionist" give you options when you pick up--before the call is connected to you.

AFAIK it is the most flexible package on the market.
