Default Looking for Contract info

Elsewhere I have been discussing a new site that I have just won the contract for. Problem is, I've never done this before and have no idea about a contract. Before I have just worked a set number of hours. This time I have accepted a lump sum for building and launching the new site plus a maintenance fee per month for the next 6 months with a review at that time to sort out what should happen in the future.

Is there such a thing as a simple generic contract that I might be able to modify and use for my purposes?

Any ideas on what things should be spelled out in such a contract?

Things that I should avoid committing to?
