If this wasn't a list you could use css to style the heading.
<hx>Some heading</hx>
<p>Some text</p>
<cite>C.S. Van Winkle: <b>The Printer’s Guide; or an introduction to the the art of printing</b>, New York.</cite>
CSS for the background and padding of the heading (you know what else you want to do with it):
hx {
    background:white url(filename.ext) no-repeat top left;
    padding:40px 0 0 0; /* adjust until it looks like you want it to */
